12.1 Phimonsis
      This is usually a congenital narrowing of the prepuce so that it cannot be drawn back over the glans. This can also occur in the adult due to chronic infection of the prepuce leading to narrowing of the prepuce.

      This operation may also be indicated when requested by parents for religious reasons.

Testis Urethra Urethral strictures

11.4 Urethral Stricture
      Most strictures met with in surgical practice are as a result of inflammatory disease of the urethra due to :
*     Uretheritis
*     Peritherial abscess
*     Tuberculosis
*     Post catheter or instrument luritheritis
*     Post traumatic uretheritis
*     Non-specific urethrites.

11.4.1 CLINICAL
      In the early stages of the inflammation the patient complains of burning and pain on muturation, accompanied by discharge later. There is straining to empty the bladder there may be palpable tenderness or swelling along the urethra.

11.4.2 TREATMENT
      After the inflamation has settled the patient has symptoms similar to those of enlargement of the prostate.
      Using calibrated dilators these strictures are dilated. Difficult strictures may need a urethroplasty

Testis Urethra Trauma 2

11.3 Rupture Of The Membranous Urethra
      Intrapelvic rupture of the urethra occurs in the membranous portion near the apex of the prostate, usually as a result of a fracture of the pelvis.

11.3.1 CLINICAL FEATURES
      Most commonly, this injury occurs in road traffic accident; crush injuries and falls, there may be in addition, injury to the head, thorax, abdomen or fracture of the long bones.
      The patients are in a state of shock from blood loss and may be unconscious. Careful assessment of the whold patient must be undertaken. There may be blood at the external urinary meatus associated with no passage of urine, the bladder may be palpable.
      If there is an intra-peritoneal ruputre of the bladder, signs of peritonitis will soon develop, but if the rupture is extraperitoneal, the diagnosis is often more difficult .


      With extraperitoneal rupture of the bladder, there is some suprapubic tenderness and perhaps a little dullness to percussion.

11.3.2 TREATMENT
      Blood loss and shock are treated aggressively, antibiotics are given.
      A suprapubic cystostomy should be performed and the bladder drained in this way.
      Endoscopy is done three weeks after the injury.
      An open procedure to re-establish continuity and the placement of a urethral catheter is advocated.

      If the prostate is floating, the rupture is complete. The older classical method-railroading a catheter across the gap is attempted.

Testis Urethra Trauma 1

2 Trauma
      Two distinct classes of rupture occur those in the bulbous urethra or the membranous urethra. They may be complete or incomplete. Rupture of the Bulbous urethra is more common accident and the patient has a history of a fall on a projecting object. The presentation is with urethral haemorrhage, a perineal haemaotma and retention of urine. The patient should be told not to try to pass urine. No attempt should be made to catherise him until he is fit to be taken to an operation theatre. Suitable dose of analgesic is given and a course of chemotherapy started.

11.2.1 TREATMENT
      Under strict asceptic precautions, an attempt is made to pass a catheter.
      If the catheter passes, it should be left in position for forty eight hours. A perineal haematoma may be drained and the midline wound packed

Testis Urethra Male Urethra Hypospadias



11.1.3 Hypospadia
      Hypospadias is a congenital malformation and occurs once in every 500 males. External meatus can be situated at some points on the under-surface of the penis or in the perineum. It may be called a glandular hypospadias or a coronal hypospadias depending on whether opening is on the glans or the shaft of the penis 



11.1.3.1 TREATMENT

      Plastic operation is required to reconstruct a new passage from the existing opening to the glans penis and to correct the deformity of the penis (corded)
